Integrated Care combines behavioral health and physical health services to provide a more holistic approach to care. There are many ways to integrate services. However, few organizations bring pediatric services to behavioral health and social service settings.

This webinar will examine the collaboration between Strong Children Wellness and The Child Center of New York, a fantastic case example of cross-organization collaborative care.  The organizations provide pediatric primary care and behavioral health services to youth and families.  During this webinar, their successful model and lessons learned will be discussed. Webinar participants will gain a better understanding of how to integrate services within their work There will be time for a Q&A to also support participants with a deeper understanding.

About Our Presenters: 

Strong Children Wellness recognizes that the traditional way of delivering medical care is often insufficient to meet the dynamic needs of children and families and has partnered with established community-based organizations to integrate primary care services into their wide array of existing support services and programs. The organization’s integrated and virtual care models allows them to expand the range of services that they provide, and streamline access to on-site specialty health and psychosocial service providers to prevent poorly coordinated care. The collective expertise of our physicians includes delivery of high quality clinical care, integrating mental health into primary care, implementing innovative programs to address social determinants of health and childhood trauma, and forging strong clinic-community partnerships.

The Child Center of NY, founded in 1953 as a children’s counseling center in Queens, is a large social service organization with more than 50 locations and 100 programs in NYC’s most under-served communities. Each year, The Child Center, serves more than 37,000 children and families through programming in 6 critical areas, including Early Childhood Education, Health Homes and Integrated Care, Prevention and Family Support, Residential Services, Youth Development, and Behavioral Health. The Child Center takes a holistic approach to supporting the well-being of children and works collaboratively with community partners to extend their reach.
